Testicular pain can be due to a variety of reasons. Pain may be acute or chronic.
Torsion of the testes is due to a reduction in blood flow to the affected testes and normally affects boys and younger men. The pain normally starts suddenly and is severe in nature. Urgent review is required to ensure the blood supply is restored as soon as possible. Restoration of normal blood supply to the testicle may not be possible after 12 hours. Restoration normally requires surgery under a general anaesthetic. If the testes can be salvaged it is fixed in place to avoid future twist. The normal side is also fixed at the same time as it is at risk of twisting in the future. If the affected testes cannot be saved it is removed (orchidectomy)
