If you leave contact details on our feedback form, we will not share those details with any third party. We do not accept e-mail communications from patients or potential patients because of security and confidentiality issues concerning medical information over the internet. We do not use cookies. Our website hosting service automatically collects information on the IP address (a code that identifes your computer’s link to the internet), pages visited, times of day, and types of browser used by visitors to the site. This information is used in aggregate form only. There is no intention to obtain or use personal data by this means. If you have a comment or complaint about the content of this site please use our email as listed. We will not enter into any e-mail correspondence about individual cases.

Terms and conditions for failure of payment for clinic attendance or late cancellation of clinic.

Please note that for insured patients the contract for payment remains between the patient and Asif Raza.

Asif Raza’s secretary may ask for advance payment ahead of the clinic appointment by debit / credit card payment to secure a clinic place.

The patient is liable to pay any shortfalls not covered by their insurance company.

If the patient states they are covered by an insurance company when they initially register with Asif Raza but there was no valid insurance cover at the outset the patient and not the insurance company are liable to pay Asif Raza s fees.

Please confirm before attending your appointment with Asif Raza that you have  valid membership and authorisation to see Asif Raza or have treatment.

If no valid insurance cover is in place you will be liable for all costs incurred plus any additional payments for late payment or debt recovery.

Please note that the contract is between the patient and Asif Raza and that the responsibility of payment is that of the patient to Asif Raza even if a valid insurance and authorisation is in place.  If insurance cover is in place the Insurance company will normally be billed  directly rather than the patient by Asif Raza for payment. However the patient is liable to pay any shortfalls not covered by their insurance company.

Clinic cancellation:

If the patient wishes to cancel their clinic this must be done by email and or text message to Asif Raza’s secretary and not the hospital or clinic.

  1. Appointments can be moved or cancelled free of charge providing the request is made more than 24 hours prior to the appointment.
  2. If a patient should cancel their appointment within 24 hours of the appointment, they will be charged in full.
  3. Cancellation fees will be issued as fee invoice to the patient and are to be paid in full within 14 days of the date of the issued invoice.
